Any patients who are seen in the Practice now need to wear a face covering or mask if asked to attend an appointment. During your visit, our Toilet facilities will not be available.

We are now offering a new Video Consultation Service and to use this service you will need to be registered for Patient Access. During these uncertain times, the best way to register for this service is by setting up online as below. If you are unable to do this you can download a form and return to us in the post or email.

Please note any digital image submitted as part of an online consultation, will be stored within your clinical notes.

Patient Registration:  If you are registering as a New Patient, you will need to complete a GMS1 (purple form) and a New Patient Form according to your age.  We can not process any New Registrations without both of these forms.

Making an Appointment

You can make an appointment with any one of the nurses or healthcare assistant at the Practice by:

  • Calling into reception
  • Contacting us by telephone
  • Booking Online

Please note: For blood tests, when you book an appointment you will need to come in before 10:30.

If you are booking any of the appointments as listed below you will need to book a double appointment. As you are only able to book a single appointment on our Internet Booking Service, you will need to telephone the surgery to book your appointment.

If you would like to speak with the Nurse for advice, please ring and leave your details and the nurse will call you back when they are free (usually at the end of Clinic).

Cancelling an Appointment

If you are unable to keep your appointment for any reason and need to cancel please let us know as soon as possible so that it can be offered to another patient.
