As We Come Out of Lockdown:
For any new Patients that wish to register we are encouraging you to download the forms, complete and submit online. A signed, scanned application or picture of a signed application emailed to the practice is acceptable. If this is not possible, you may print and complete manually and post through the Practice mailbox where we will process your Registration without Identity Checks. If you are unable to complete the form yourself then please contact us and we will be happy to do this on your behalf. If you do not have access to a printer please call the Practice and express your interest to register and we will then prepare the forms for you to collect.
New Patient Registration
We welcome new Patients from within our catchment area and are happy to register you. It’s free to register. Please refer to the map in our booklet or use the catchment tool here to check practice area
If you would like to register with us, you can either come into Reception to collect a Registration form and GMS1 form or you can download these documents from our Website or we can email them to you.
When downloading, ensure you are completing the correct Registration Form as this is according to age Groups. All new Patients must complete the GMS1 form which is included in each Registration Pack. Please detail as much information as possible and all details on the GMS1 form need to be completed before we can register you.
You do not need proof of address or immigration status, ID or an NHS Number but it might help for one of the following reasons:
- To confirm you live in our catchment area
- To confirm we are registering the correct person
- To check the Health Authority have the correct information
- To prevent fraud (to stop people registering with many practices)
However if you do not have any ID we will accept your registration, it might help if you have one or more of the following:
Photo Identification:
- Passport
- Birth Certificate
- HC2 Certificate
- ID card for European Countries
- Rough Sleepers’ Identity Badge
- Hostel or Accommodation Registration or mail forwarding letter
If you are homeless, you can give a temporary address, such as a friend’s address, a day centre or the GP Surgery address.
All Patients are registered with a named GP: Dr Larissa Tate. You will be able to make an appointment with any Doctor of your choice.
Once you have registered with the Practice, we will offer you a New Patient Health check with the Nurse or Healthcare Assistant.
We offer our new Patients the facility to book all appointments online with our Doctors’, Nurses’ and Healthcare Assistant using our online services. You can register for this service when you join the Practice. These forms are included in our Registration Packs and are Patient Access forms.
You will need to download and print, or save> edit> email us the New Patient Pack for each person wishing to register and ensure that you have the correct age range. To assist you we have included the GMS1 form (Family Doctors Services Registration) in the Packs.

2021 Registration Pack for Under 5's
For children under the age of 5 years, we will need you to complete a Health Form for our Health Visitors which you can download and print by clicking on the link above.
When you have registered with the Practice, we will offer you a New Patient Health check with the Nurse.
