Jointly App for Carers

The National Commitment to Carers Team procured the Jointly App which helps to support the long term plan objectives for unpaid carers. The app provides support to ‘circles’ of carers and also supports: 

  • Registering with your GP (and eligibility for flu, health checks etc)
  • Contingency Planning
  • Young Carer Top Tips

The Carers Centre for Brighton and Hove is leading the delivery of the Carers Hub.  They support adult carers and young carers in the city.

Are You A Carer?

We are interested in identifying carers, especially those who may be caring without help or support.  Caring for someone is an important and valuable role, which is often a 24-hour job that can be very demanding and isolating.

An unpaid carer is a person of any age, includes young carers, who provide unpaid support to a partner, family member, or friend who could not manage without their help and support due to their illness, frailty, disability, a mental health problem, an addiction, or it could be someone who has dementia or a learning disability.

There is no one size fits all, you can be offering support a few hours a week, or round the clock care, you may be living in the same house with the person you care for, or you maybe be living separately

As a carer, you are entitled to have your needs assessed by Adult Care Services.  A carers assessment is a chance to talk about your needs and ways you could receive help. It also looks at the needs of the person you care for. There is no charge for an assessment.

Who is A Young Carer?

Young carers are children and young people under 18 years old who help to care for someone in their family.  Most young carers help to care for one of their patients or care for a sibling and help with extra jobs around the home, with their family member would find difficult without their support

 What you can do to look after yourself

Ask a member of staff to refer you, speak with our Carers Champion.  You can also download a Family  Friend Carer Form here and send to the Practice:
